CAPS & Maple City is excited to welcome your patient referral as a preferred referral partner. Please find below the simple process to submit a patient referral as well as FAQ’s to the process.

Submitting a Patient Referral

Utilize any of the following contact points to submit a new patient referral

1. Call the CAPS & Maple City Access Phone Line: 574-534-3300

2. Use the contact form here. 

3. Send an email to the referrals email: referrals@mchcc.org

4. Send a fax to: 574-375-4146

Once a referral has been submitted, you can expect the following steps to take place

1. Once a referral is received, we will make contact with the patient via phone call

2. Patient information is gathered during that initial contact, and the patient chart is created.

3. A registration appointment is made.

4. Patient comes in-person with needed documents (insurance, income information, ID, information to request records from previous doctors) for the registration appointment. All required paperwork is filled out. The patient leaves with a scheduled new patient appointment. This is currently required as an in-person appointment.

5. Patient comes in for new patient appointment with their Primary Care Provider.
